1. Students entering Pre-School or Kindergarten must have a birthday on or before October 1st of the current school year.

2. Students entering Pre-School (3 years old) must be potty trained.

3. Students entering 1st-6th grade must have a letter of acceptance after passing the entrance exam.
Students entering 7th-8th grade must fill out an enrollment questionnaire as well as pass the entrance exam before receiving a letter of acceptance.

4. A copy of the student’s birth certificate and shot record.

5. A current physical for students entering K3, K4, K5, 5th, 8th, or new to the Illinois school system.

6. A current dental exam for students entering K5, 2nd, and 6th grades. Exams prior to November 15th of the previous year will not be accepted.

7. All required paperwork completed, registration fee paid, and FACTS payment agreement turned in.

8. School handbook agreement signed acknowledging parent and student have read and agree to contents.
